We are looking for an enthusiastic, self-motivated Community Matron  or development Community Matron , to join our Integrated Community Team based in Chesterfield.

Community Matrons are an essential part of the Integrated Community Team - working closely with GPs in the locality to a ‘caseload management’ style.

We want to recruit clinicians who are able to support and empower patients to manage their long term conditions whilst working to achieve realistic goals that are patient-led.

You will receive regular clinical supervision and peer support to enable you to develop in your role.

We will positively consider applicants who fulfil the Person Specification, with the exception of non-medical prescribing and advanced clinical decision making/patient examination skills.

Applicants without these criteria must be willing to accept a Band 6 Trainee Matron role until successful completion of the courses and learning required for the role.
